Hogyoku Fragment #4 Location (Shinjuku Island)

Hogyoku Fragment #5 Location (Slime Island)
Run the left edge of Slime Island until you hit the Rimuru Mastery trainer, then double back. Jump onto the central tree and clamber onto the branch—fragment #5 sits there. Sometimes these fragments are as obvious as a neon sign in a blackout; other times they hide in plain sight.
Hogyoku Fragment #6 Location (Judgement Island)
Teleport to Judgment Island and run the right edge toward the iceberg spawn. Fragment #6 rests on a nearby ledge—easy pickup if you hug the coastline. Finish this one and head back to Hueco Mundo without stopping to fight world bosses unless you’re certain of the timer.

Once you’ve touched all six fragments, talk to the Hogyoku NPC again. A portal pops up beside him and remains open for 120 seconds—get in, or you’ll have to repeat the sequence. After your first successful trip, you can teleport to Soul Society from any in-game Teleport Portal on the map.
